Firstly, caffeine drains your immune system by stressing your adrenal glands. Every time you drink coffee you are stimulating your adrenals to produce adrenaline. More recently, flexible ureteroscopes combined with small holmium laser fibers have allowed retrograde access to the kidney for stone destruction. PNL was developed and popularized in the 1. Access to the kidney is obtained under ultrasound or fluoroscopic guidance, and the tract is typically dilated to 2. F. Through this tract, a rigid nephroscope is introduced to visualize the stone and collecting system and to guide fragmentation. Energy sources for this purpose include ultrasonic, electrohydraulic, and pneumatic lithotrites and laser. Both laser and electrohydraulic ureteroscopic lithotripsy are possible and can significantly fragment staghorn calculi. Case reports describe a synchronous bidirectional technique that combines percutaneous nephroscopy and retrograde intrarenal surgery to successfully treat complex, branched staghorn calculi, lessening the need for multiple flank punctures. As with SWL, the passage of a large stone burden requires an indwelling ureteral stent, and multiple treatments may be required. For both monotherapy SWL and retrograde ureteroscopic lithotripsy, rendering the patient stone free is difficult, especially in the setting of dilated collecting systems and dependent lower pole calyces. Surgical principles. Although multiple surgical approaches to staghorn calculi are available, several principles must be kept in mind. First, complete removal of all stone material is the goal of any procedure. Simple debulking does not prevent future infections, stone formation, or impairment of renal function. Second, the patient should be counseled that multiple interventions may be required. If ureteroscopy or SWL is the primary treatment modality, the need for subsequent SWL, ureteroscopy, or PNL must be discussed. Use of combination therapy is a reasonable approach to ensure removal of all residual fragments. An example of this is the so- called sandwich technique, with initial PNL followed by SWL and then second- look PNL. Patient outcomes have been demonstrated to improve with increasing PNL experience. Third, the immediate use of adjunctive measures can be considered. Postoperative oral acetohydroxamic acid (AHA) and antibiotics may delay the regrowth of struvite stones. In addition, direct irrigation of the collecting system is possible through the nephrostomy tube after PNL. Lavage chemolysis for residual fragments consists of acidification of the urine with solutions such as Suby G or hemiacidrin (Renacidin). Although in vitro and in vivo data support some efficacy of direct acidification and ion exchange of stone calcium for magnesium, caution must be used when performing irrigation to ensure sterile urine, low intrarenal pressure, and normal serum magnesium levels. Hypermagnesemia (and associated toxicity) is more common in patients with compromised renal function. Recommendations. How to Be a Responsible Cat Owner While cats are generally low maintenance pets compared to dogs, they still require a lot of care and attention. Despite the name fisher, the animal is not known to eat fish. The name is instead related to the word fitch, meaning a European polecat (Mustela putorius. The optimal diet for a cat is a properly formulated raw, home- cooked or grain- free canned diet. Never feed dry food. Cats shouldn’t eat dry food; even the grain- free dry varieties are too high in carbohydrates. And contrary to the myth that just won’t die, dry food does not clean your cat’s teeth. Most cats don’t chew their kibble long enough for any of the scraping action that is the theory behind this myth to kick in. What little they do chew shatters into small pieces. Some pet food manufacturers offer a “dental diet” that is made up of larger than normal sized kibble to encourage chewing, but in my years at veterinary practices, I’ve seen many cats swallow even those larger size pieces whole. If you have a rash or lesion on your skin which is suspicious for a certain condition, does not respond to therapy prescribed by your doctor, or the cause of which is unknown, your doctor may perform or order a biopsy of the involved area of skin. This can be done by using local anesthesia and removing a small piece of the area with a razor blade, a scalpel, or a small, circular blade called a “punch.” The specimen will be sent to the lab to look for evidence of conditions such as infection, cancer, and inflammation of the skin structures or blood vessels. Surgical biopsy. Sometimes a patient may have an area of concern that cannot be safely or effectively reached using the methods described above or the results of other biopsy specimens have been negative. An example would be a tumor in the abdomen near the aorta. In this case, a surgeon may need to get a specimen using a laparoscope or by making a traditional incision. Any medical procedure that involves breaking the skin carries the risk of infection or bleeding. However, as the incision is small, especially in needle biopsies, the risk is much lower. Biopsies may require some preparation on the part of the patient such as bowel prep, clear liquid diet, or nothing by mouth. Your doctor will instruct you on what to do before the procedure. As always before a medical procedure, tell your doctor what medications and supplements you take. You may need to stop taking certain drugs before a biopsy, such as aspirin or nonsteroidal anti- inflammatory medications. After the tissue sample is taken, your doctors will need to analyze it. In some cases, this analysis can be done at the time of procedure. More often, however, the sample will need to be sent to a laboratory for testing. The results can take anywhere from a few days to a few weeks. Once the results arrive, your doctor may call you to share the results, or ask you to come in for a follow- up appointment to discuss the next steps. If the results showed signs of cancer, your doctor should be able to tell the cancer’s type and level of aggression from your biopsy. If your biopsy was done for a reason other than cancer, the lab report should be able to guide your doctor in diagnosing and treating that condition. If the results are negative but the doctor’s suspicion is still high either for cancer or other conditions, you may need another biopsy or a different type of biopsy. Your doctor will be able to guide you as to the best course to take. If you have any questions about the biopsy prior to the procedure or about the results, don’t hesitate to talk with your doctor. You may want to write down your questions and bring them with you to your next office visit. How To Tell If You Have Gastritis. Gastritis is an inflammation of the stomach lining or mucosa. The inflammation may be caused by viral infection, alcohol, smoking, certain drugs, poisoned food, or stress. There are many forms of gastritis. Erosive gastritis can lead to significant bleeding of the stomach and ulcerations. Gastritis may be acute or chronic. It can occur with any major physical stress such as severe illness, surgery or burns, and therefore is referred to as stress- induced gastritis. Medications that are well- known stomach irritants can contribute to gastritis. These include aspirin, nonsteroidal agents such as ibuprofen and pancreatic enzymes. Excessive alcohol ingestion can also cause gastritis and erosions. Sites like alcohol- rehab- nj. Don't eat solid foods on the first day of the attack, give your stomach a rest and drink liquids only, milk or water are preferred. Add bland foods to your diet slowly and as tolerated (cooked cereals, bananas, rice, potatoes, toast) and avoid greasy, spicy foods. Chronic gastritis can be treated only by eliminating the causative factor, for example, alcohol, smoking, or highly spiced or other foods that are difficult to digest. Antacid drugs are recommended for the treatment of chronic gastritis. The classification of gastritis can depend on the region or function, predominantly involved.
